  2. * - period cancer deaths were obtained from the civil registry for 2008-2017; cohort cancer deaths were obtained from the cancer registry database for patients diagnosed in 2008-2017.
  3. †- Mortality to incidence ratio was based on the number of deaths from the civil registry.
  4. ‡- all DCI cases were excluded, including those with clinical/pathological information
