1. Development of a survivorship care plan

Participants were asked whether they developed with their care team, or were provided with, a cancer survivorship care plan (SCP). SCP was defined to the participant as “A summary of your treatment, together with recommendations for follow-up care and leading a healthy lifestyle.”

Tell me whether you developed, or were provided with, a survivorship care plan with your oncologist or physician after completing cancer treatment.

2. Communication of survivorship care

Participants shared their experiences with communication with their healthcare providers after completing curative-intent chemotherapy.

What conversations did you have with your healthcare team about what you could expect as someone who has completed chemotherapy? What are your impressions of how communication is/was going between your oncology team and primary care doctor?

3. Survivorship care challenges

Participants shared any challenges they may have experienced during their post-treatment phase (e.g., side effects, transportation, etc.)

Did you experience any challenges during post-treatment cancer care? If yes, tell me about them.

4. Supportive Network

Participants shared whether they have a supportive network/individual, and if they do, their role during cancer treatment and after the conclusion of cancer treatment.

Please tell me about your social support network. What has been the role of family and/or friends during cancer treatment? How about after your cancer treatment ended?

5. Recommendations to survivorship care

Participants shared recommendations to improving their post-treatment survivorship care.

Being a person who has completed chemotherapy, do you have any advice for how to improve your survivorship care experience?
