  3. aPrimary care physician includes physicians in general practice, family medicine, internal medicine, and obstetrics/gynecology. APRN indicates Advanced Practice Registered Nurse which includes certified nurse midwives (CNM) and nurse practitioners (NP); PA indicates physician assistant.

  5. cOther healthcare providers include mental health professionals such as a psychiatrists, psychologists, psychiatric nurses, or clinical social workers.
