STM is the only manifestation of a small cell lung cancer. A 74-year-old woman presented with a 1-month history of a subcutaneous mass on the right side of her waist, which was confirmed as metastatic small cell carcinoma on biopsy. MIP (a) of 18F-FDG PET/CT showed a soft-tissue mass in the lower lobe of the right lung (arrowheads), with elevated FDG uptake (SUVmax = 8.4). MIP (a), chest axial images (b-d), and pelvis axial images (e-j) revealed multiple nodules and masses throughout subcutaneous tissue and skeletal muscle (short arrows) with increased FDG uptake (SUVmax = 7.5). Subsequently, lung biopsy confirmed small cell lung cancer of the right lung. After 11 months of palliative chemotherapy, the patient died of respiratory failure
