  1. † Thickness of the lesion was also used to categorise disease stage at diagnosis for melanoma [T3 (thickness = 2.01-4.0 mm) and T4 (thickness > 4.0 mm) stages were grouped with ‘spread to regional lymph nodes’ as ”regional”]. Since thickness data prior to 1983 were considered unreliable, we used melanoma data from 1983 onward for the stage-specific survival estimates.
